All NOVA students should have blank NOVA forms ready for the DME, and all the necessary evaluation(s) on NYCOM/NYIT from attending and resident physicians. All LOGS should be presented. All evaluation forms for the rotation(s) should also be completed. Each month requires 30 questions in email and PRINT form which must be ready at interview time. | ||
